Acute filarial funiculo- epididymo- orchitis
Aetiology: is due to lymphatic obstruction by filarial parasite (Wuchereria Bancrofti) with secondary streptococcal infection (blood borne).
PATHOLOGY
Four pathological types are described:
Acute catarrhal type:
The spermatic cord is acutely inflamed. The cord tissues are infiltrated by inflammatory exudates and cells. As a result; the cord is thickened. The cord structures are matted together and feel like a lead pipe. The cord structures including the vas can not be felt separate.
Acute suppurative type
is similar to the non-filarial type but the swelling is more extensive and reach to the inguinal canal. Suppuration occurs, both diffuse cellulites and abscesses.
Acute gangrenous type: The arteries and veins are thrombosed, and this cause sloughing of the testis.
Acute fulminating type: Is like the gangrenous type but with more severe toxaemia that could be fatal even before the local pathology is well established.
